Chapa-De Indian Health Auburn Grass Valley | Medical Clinic
Chapa-De Indian Health is a full-service health center offering a variety of services. In those services, we use a “Patient-Centered Health Home” approach. That means our providers put you first! We help patients set their own health goals and then work with them to achieve those goals. Our providers work together across departments to focus on a patient’s overall health, not just a single issue.

Medical Care
We offer excellent primary medical care for the entire family, from newborns to seniors. This includes treating new and ongoing health concerns, as well as preventative care services.
Dental Care
Dental care includes services such as cleanings, fillings, exams, and more. We also perform limited root canals and oral surgeries.
Optometry
Optometry services are offered at our Auburn health center. Services include exams, treatments, ordering and adjusting glasses, and more.
Behavioral Health
We have teams of psychiatrists and therapists at our health centers. Our providers want to help patients meet their goals for mental and emotional health. We typically see patients once a month for a variety of mental health concerns.
Pharmacy
We are proud to have in-house pharmacies at both of our health centers. We can provide our patients with many reduced-cost medications.
Diabetes Program
Chapa-De patients can participate in group classes and education programs that focus on the prevention and management of diabetes.
Prenatal Care
Prenatal health services are offered at both of our health centers. In addition to regular appointments with a doctor, we have our Baby Luv Program. This program helps women get the education and services needed for a healthy pregnancy and a healthy baby.
Telehealth Program
Chapa-De’s Telehealth program offers a wide range of specialty care services through a live two-way video feed. Our Telehealth program lets patients see specialists without leaving Chapa-De.
Addiction/Substance Use
We offer non-judgmental and compassionate care for all addictive diseases. The Chapa-De team provides routine screenings, education, brief interventions, and referrals to treatment.
PACE – For Chronic Pain Patients
The PACE classes at Chapa-De teach chronic pain patients new ways to manage pain and increase their quality of life.
Health & Wellness Classes
We host free health education classes each day on a wide range of topics. The classes aim to empower patients to be well and have healthy lives. Classes include education about medical topics, as well as exercise classes.
