What Local Medi-Cal Members Should Know.

Chapa-De Indian Health is excited to welcome Partnership Healthplan to our area! Partnership expanded to more Northern California counties effective on January 1, 2024. This includes Placer, Nevada and Yuba counties. Medi-Cal members who were covered under Anthem or California Health & Wellness, may now be covered by Partnership.
The goal of this change is to offer a wider network of providers for those on Medi-Cal. We hope this will lead to an improved quality of care for thousands of people across Northern California. What Local Medi-Cal Members Should Know
Partnership ID Cards
Due to processing delays, members may not receive their new member packets and Partnership ID cards in the mail at the very start of the new year. If you have appointments scheduled at Chapa-De in the first weeks of January, we highly encourage you to take these steps:
- Sign up for the Partnership Membership Portal online at https://member.partnershiphp.org/
- Log-in and verify you are assigned to Chapa-De as your primary care provider (PCP)
- If you are not, please call Partnership Members Services at (800) 863-4155 to have it fixed
If you will be seeing a specialist outside of Chapa-De in the first weeks of January, we also suggest printing out a copy of your membership card and taking it with you to your appointment. No printer at home? Take a photo or screenshot on your phone.
Rides to Healthcare Appointments
Free transportation is a critical benefit for Medi-Cal members. Partnership has a specially trained team to help schedule rides and ensure you can get to your appointments. To schedule a non-emergency ride, please call:

Providers and Services at Chapa-De
Chapa-De is a contracted provider with Partnership. We are in the Partnership network, and both our Auburn and Grass Valley health centers will accept Partnership. This means that Chapa-De can continue to provide your Medical, Optometry, and Behavioral Health care. There will also be no changes to your coverage or benefits for other services like Pharmacy and Dental. We are working closely with Partnership and will help address any problems that come up for our patients during this transition.
Providers and Services Outside of Chapa-De
If you receive specialty care or services outside of Chapa-De, Partnership will honor all treatment authorizations previously approved by the prior Medi-Cal Managed Care Plans. You should keep all scheduled appointments. Partnership will also work with you to make sure you receive the care you need moving forward. You may be able to continue seeing your current specialist(s) or Partnership will help you get set-up with a similar provider. If you are a patient at Chapa-De, our team will also support you throughout this process.
