We call our approach “Patient Centered Health Home”, but it is really just taking care of you the way you should be.
WHAT HAPPENS AT A WELL CHILD CHECK?
Well Child Checks are important because kids grow and develop fast! At each Well Child Check, your child’s Care Team will measure things like height and weight, check their vision and hearing, listen to their heart beat, and screen for other important things. By seeing your child regularly, your child’s Care Team will be able to see how they are growing and developing over time, and make sure that they are right on track.
Chapa-De’s Care Teams consist of everyone from a Doctor, to Nurse Practitioners, Registered Nurses, Medical Assistants, and Patient Service Representatives, all working together to give you the attention and care you deserve at each visit!
Well Child Checks are also a good time to talk with your child’s Care Team about any concerns you may have, any problems you have noticed, and to learn about healthy nutrition and other easy things to do at home to help keep your family healthy and out of the Emergency Room.
MEDI-CAL MAKES THIS POSSIBLE
Remember, with the new Medi-Cal law, SB 75, that went into effect in May of 2016, any child under the age of 19 can qualify for full scope Medi-Cal, regardless of immigration status! Medi-Cal will cover Well Child Checks. Medi-Cal will also cover many other health services for your family.
