Why Understanding Health Information Is Important
Chapa-De Indian Health Auburn Grass Valley | Medical Clinic
At Chapa-De, we know the best outcomes happen when patients are empowered to take an active role in their health. Empowered patients take the recommendations and education offered by their care team and use it to make informed choices. October is Health Literacy Month and we have tips to help you feel in control of your care and wellbeing.
What is Health Literacy?
Health literacy is the degree to which individuals can gather and understand health information that they can put toward making the best decisions for themselves possible. This month is a time meant to empower individuals to seek out the necessary information they need in order to make informed health-related decisions.
Some health literacy resources include:
- National Action Plan to Improve Health Literacy contains 7 goals that will improve health literacy and strategies for achieving them
- MyHealthFinder is a prevention and wellness resource that includes easy to understand evidence-based health information

How does Chapa-De Promote Health Literacy?
At Chapa-De, we follow a healthcare model that is both patient-centered and integrated. The patient is the central focus of our services and processes, and they will receive care from a highly skilled team of healthcare professionals. We want our patients to be active participants in their care, and our team is here to support you in that.
We want you to have consistent communication with your primary care provider and the healthcare team that supports you. We will do our best to make sure that you see the same healthcare team at every visit, so that you become comfortable with us and we understand you. We want to help you set your own health goals, and then help you work to accomplish them.
Do not hesitate to reach out to your care team if you need further information about anything to make the best decision for yourself and your health. We understand that medical jargon and processes can be confusing, and we are here to help you navigate whatever you may need. Let’s continue to stay educated together so that we can keep you healthy!
