Diabetes Programs at Chapa-De
The diabetes department at Chapa-De offers a variety of programs. They are designed to help with the prevention of or management of diabetes. These programs include classes on health and wellness as well as fitness and weight loss.
- Diabetes Prevention Program: a year-long program that helps patients to avoid the onset of diabetes and to live healthier lives
- Diabetes Wellness Program: diabetes program staff work with your medical care team to help you achieve your goals for health and wellness through education and support
- Diabetes Support Groups: monthly groups include a Type 1 group, a Type 2 group, and an AIAN group
- Exercise Groups & Classes: exercise classes allow each participant to workout at their own level under the professional guidance of our Certified Personal Trainer

A New Diabetes Prevention Series Starts This Month – Sign-Up Today!
Our free Diabetes Prevention Program (DPP) can help participants reduce their risk of diabetes by as much as 58% – or 71% for those 60 years and older. These classes are open to everyone, especially those that have been told they have pre-diabetes or are at risk of developing type 2 diabetes. Any Chapa-De patient age 18 and over is welcome and will join an interactive online program that you can do from the comfort of your home. Participants receive support from Chapa-De Lifestyle Coaches Margreet Adriani and Jay Sanchez. Margreet is a nutrition specialist and Jay is a certified personal trainer.
