At Chapa-De, we are always looking for ways to be there for you when you need us most. After all, your health is our mission. One of the ways we provide you with convenient access to high quality, compassionate care, is through our hours of operation.
Patient Survey about Expanded Hours
We want to hear from you! We are conducting a brief, 4-question survey from September 19th to October 31st to better understand your preferred days and times for appointments. This will help us understand if we need to expand our hours to serve your needs.
Current patients will receive email and text message invitations to participate in the survey, or you can click HERE now to participate. Prior patients and community members interested in receiving care at Chapa-De are also welcome to use this link to participate.
Why are we doing this?
Have you had to delay therapy for a child due to school hours? Have you missed a dental cleaning because you could not leave work? We understand it can be hard to attend appointments during traditional office hours. We are open to making changes to our days and hours of operation in the future. We want to hear from you so we can make adjustments accordingly.
Your response to this survey will provide important information to help us make these decisions. If you are a current patient or interested in being a patient in any of our departments, please respond to our survey so that your voice is heard.
A committee at Chapa-De will review the results of this patient survey. We will then conduct a survey with our staff and will use the results from both surveys to determine what changes may be necessary, if any. This process will take time and changes, if necessary, may not take place until 2023.
