Welcome to Our New Health & Wellness Department!
We have exciting news! Our department has a new name and a new focus. What used to be our Diabetes Department is now the Health & Wellness Program. Don’t worry, we’re still here to help you with diabetes. And now, we’re also here to help you with much more!
Why the Change?
More and more people are looking for ways to live healthier lives and not just manage one condition. We listened. That’s why we’re expanding our services to support your whole-person health. Whether you want to eat better, move more, lose weight, lower stress, or manage a health condition, our team is here for you.

You’ll still see the same friendly faces you know and trust. Our team of caring professionals is here to support you. We remain committed to supporting you with comprehensive diabetes management, lifestyle coaching and nutrition education.

We offer many ways to support your health and wellness. Our team can help you learn about:

We offer one-on-one appointments and group classes. Whether you like learning on your own or with others, there’s something for everyone!
Meet Your Health & Wellness Team
Our department includes several caring and highly trained professionals who are here to support your goals:
- Registered Dietitians (RDs): Available to any Chapa-De patient who wants help with their diet or nutrition. They offer Medical Nutrition Therapy and personalized plans to support your unique health needs.
- RN Diabetes Case Managers: These nurses work with patients who are living with diabetes. They can help with self-care strategies, understanding medications, and lowering health risks.
- Nutrition Specialists & Lifestyle Coaches: Available to patients with pre-diabetes, those wanting to lose weight, or anyone looking to eat healthier and feel better overall.
- Certified Personal Trainer & Lifestyle Coaches (Auburn Health Center only): Offer one-on-one training sessions and fun group exercise classes to help you move more and meet your fitness goals.
- Medical Assistants: Help ensure your visit and services go smoothly. They also conduct retinal eye screenings for patients with diabetes.
