In the midst of COVID-19, it might seem hard to receive the care and support you are normally accustomed to. We understand. We want you to know that Chapa-De is here for you! We want our patients to know that we are doing our best to create a sense of normalcy during an abnormal time and that includes continuing to offer many of our groups and classes.

Focusing on your health and making healthy lifestyle choices is still important. Our team is bringing groups together through group phone calls and video meetings so you can continue to access the information and support you need while also maintaining distance from others. Home may not be a place you normally associate with group learning and interaction but now it can be!

What Groups & Classes are Available?

  • Weight Loss Support Group:
    • Mondays from 4pm – 4:45pm via Zoom
  • Diabetes Prevention Program Aftercore Group
    • Every 4th Tuesday of the month from 4pm – 5pm via conference call
  • Diabetes Support Group for American Indians
    • Every 3rd Tuesday of the month from 3pm – 5pm via conference call
  • Diabetes Type 2 Support Group
    • Every 1st Tuesday of the month from 2pm – 3pm via conference call
  • Diabetes Type 1 Support Group
    • Every 2nd Thursday of the month from 3pm – 5pm via Zoom
  • Medication Assisted Treatment (MAT) Group
    • Offered every Wednesday and Thursday – call for more information

New Diabetes Prevention Program Series!

Starting on July 9th, we will kick-off a new series of our Diabetes Prevention Program. This is a free year-long program for patients with pre-diabetes. Participants will learn about healthy eating, weight loss, physical activity and stress management in a non-judgmental and supportive setting. Patients come together with others who are facing the same challenges to receive support and guidance as they make healthy lifestyle changes that last.

Patients who have completed the Diabetes Prevention Program say their lives are happier now that they’ve committed to making themselves and their health a priority. They also enjoy sharing what they’ve learned with their family and loved ones.

  • Class Details:
    • 22 interactive sessions throughout the year facilitated by our Nutrition Specialist and Certified Personal Trainer
    • Individualized support offered by experienced lifestyle coaches
    • Recipe Sharing
    • All of this is free to participants
  • Schedule
    • Core Classes: the first 6-months are designed to offer more support on a more consistent schedule. At the beginning we’ll meet weekly then taper down to every other week. These classes are on Thursdays from 5:30 pm – 6:40 pm via Zoom.
        • Once a week for 10 sessions:
          • July 9th, 16th, 23rd, 30th
          • August 6th, 13th, 20th, 27th
          • September 3rd,10th
        • Every other week for 6 sessions:
          • September 24th
          • October 8th, 22nd
          • November 5th, 19th
          • December 3rd
        •  
    • After Core Sessions: the second 6-months are focused on sustaining changes with less frequent check-ins. From January through June we will meet once a month on the 4th Tuesday of each month from 4-5 pm via Zoom.
        • Once a month for 6 sessions:
          • January 26th
          • February 23rd
          • March 23rd
          • April 27th
          • May 25th
          • June 22nd

How do I Sign Up for Classes?

Participating in classes is simple. Visit our Class Calendar on our website. Choose a class you want to join by clicking the link on the calendar. This will direct you to the class page where you can call the appropriate department’s phone number to sign up. Once signed-up you’ll receive instructions for how to join the group through a conference call or video conference.
