New with Medi-Cal: You May Be Eligible for Community Supports

There is great news out for Medi-Cal patients. Medi-Cal has expanded to now cover several new services and has a new Case Management program to help overcome challenges faced by many members. New Community Supports services can help address housing challenges, ensure members have a safe place to recuperate after leaving the hospital, and more. If you have Medi-Cal, you may be eligible to receive these services that expand beyond traditional healthcare. 

Community Supports 

There are many factors that impact our health. Based on your needs and eligibility, Medi-Cal can now provide or help pay for services to address needs outside of the traditional healthcare system. The following services are now available to eligible Medi-Cal members in Placer and Nevada County who have coverage under Partnership Healthplan of California: 

Housing Transition Navigation Services: If you are unhoused or at risk of experiencing homelessness, Medi-Cal members can receive help to find, apply for, and secure housing. 

Housing Deposits: Receive help with housing deposits, first and last month’s rent, the first month’s utilities and more to help members get established in their home. 

Housing Tenancy and Sustaining Services: Members can receive support to maintain safe and stable housing once secured. 

Short-Term Post-Hospitalization Housing: Up to six months of short-term care housing if needed for recovery after a hospitalization or for substance use recovery. 

Recuperative Care (Medical Respite): For members with unstable housing who no longer need hospital care but also have recuperative needs can receive housing, meals, transportation and more to aid in their healing. 

Respite Services: Caregivers of Medi-Cal members can receive short-term relief in an hourly, daily, or nightly basis with caregiver services. 

Personal Care and Homemaker Services: Members who need help with Activities of Daily Living or Instrumental Activities of Daily Living can receive in-home support for hygiene, meals, and more. 

Medically Tailored Meals or Medically Supportive Food: Receive meals made specific for your medical needs delivered straight to you.

Additional Community Support Services may be available to you depending on where you live and what Medi-Cal health plan you are assigned to.

How to Request Community Supports 

There are a few ways to request Community Supports, the first being through Chapa-De with one of our Care Coordinators. They can walk you through and begin the request process to get you the care you need. You can also contact your Medi-Cal Health Plan to be connected to someone who can help you get started. 

Enhanced Care Management (ECM)

Healthcare can be overwhelming to get a handle on, especially if you are dealing with complex medical conditions. That’s where Enhanced Care Management comes in to help manage not only your healthcare but also other health-related care by meeting right where you are. 

What is Enhanced Care Management? 

Enhanced Care Management is one of the many new resources available to certain eligible Medi-Cal patients.  What makes this service special is that it can help connect members with complex medical needs to both clinical and non-clinical services. When you aren’t able to manage your care due to social complications like Substance Use Disorder, housing insecurity, or even transitioning back to normal life after incarceration, a single lead care manager can help. They can coordinate physical, mental, and dental care, social services, and even advocate for you to receive the services you need. 

Enhanced Care Management at Chapa-De Indian Health 

Now at Chapa-De, we can provide Enhanced Care Management for our Medi-Cal patients with complex needs. If you are: 

  • Adults, unaccompanied youth and children, and families experiencing homelessness 
  • Adults, youth, and children who are at risk for avoidable hospital or emergency department care 
  • Adults, youth, and children with serious mental health and/or substance use disorder needs 
  • Adults living in the community and at risk going into a long-term care facility like a nursing home 
  • Adult nursing facility residents transitioning to the community 
  • Children and youth enrolled in California Children’s Services (CCS) or CCS Whole Child Model with additional needs beyond their CCS condition(s) 
  • Children and youth involved in child welfare (foster care) 
  • Adults and youth who are transitioning from incarceration 
  •  Pregnant and postpartum individuals
