Now Accepting Scholarship Applications

Each year, Chapa-De Indian Health is honored to award a $1,000 scholarship in memory of April Moore, a dear friend who served as our Board President for many years. April dedicated her life to assist in advancing American Indian people and served on the Chapa-De Board of Directors for 26 years.
We are pleased to announce that Chapa-De is now accepting applications for the April Moore scholarship for 2018. We invite all American Indian students who plan to pursue a career in a healthcare related industry to apply. In order to qualify for this scholarship each applicant must:
- Have a minimum cumulative GPA of 3.0 or above
- Be American Indian or Alaskan Native
- Plan to pursue vocational training or a college degree within a healthcare related field
- Must have attended school in Nevada, Placer, Sacramento or Sierra County in California
Along with meeting all of the qualifications listed above, each student is required to submit:
- The April Moore Scholarship Application Form | Download PDF | DOC-X
- Two (2) Letters of Recommendation
- Official Transcripts- High School and Secondary School if applicable
- Proof of AI/AN heritage
Completed applications must be submitted by March 31, to be considered for the 2018 award. Chapa-De’s Board of Directors will select the winning applicant.
