Welcome to Health & Wellness

The Health & Wellness Program at Chapa-De is here to support you! Our friendly team is ready to help you reach your goals for a healthy life. This team is specially trained to support patients to eat healthy, stay active, and manage conditions like diabetes and obesity. We invite you to learn more about your health and choices that can help you feel better. Services are available one-on-one and/or as part of a group.

This program welcomes all Chapa-De patients who receive care in our Medical Department.

Program Staff

Our Health & Wellness team members include:

Registered Dietitians

Available to any Chapa-De patient who wants help with their diet or nutrition. They offer Medical Nutrition Therapy and personalized plans to support your unique health needs.

Diabetes Nurse Case Managers

These Registered Nurses work with patients who are living with diabetes. They can help with self-care strategies, understanding medications, and lowering health risks.

Lifestyle Coach/Nutrition Specialist

Available to support patients with pre-diabetes, those wanting to lose weight, or anyone looking to eat healthier and feel better overall.

Lifestyle Coach/Certified Personal Trainer (Auburn Health Center only)

Offer one-on-one training sessions and fun group exercise classes to help you move more and meet your fitness goals.

Medical Assistants

Help ensure your visit and services go smoothly. They also conduct retinal eye screenings for patients with diabetes.

Health & Wellness Groups and Classes

We invite you to take part in our friendly and supportive groups and classes. We provide a comfortable, judgement-free zone where you can explore ways to keep or improve your health.

Diabetes Prevention Program

A year-long program to help lower your risk of developing type 2 diabetes while living a healthier life. Learn More.

GLP-1 Education Class

Learn about GLP-1 medications such as Mounjaro, Wegovy, Ozempic, and more. Whether you are already taking one of these medications or want to learn more, this class is for you.

Type 2 Diabetes Support Group

Open to all patients living with type 2 diabetes. These meetings offer a caring space to learn, share, give and receive support.

Type 2 Diabetes Support Group for American Indian Patients

For America Indian and Alaska Native patients living with type 2 diabetes. These meetings offer a caring and culturally aware space to learn, share, give and receive support.

Group Exercise Class

Our 1-hour classes focus on heart health, strength, and coordination with regular breaks. All patients aged 18 and over are welcome after a first appointment with the Personal Trainer.

Chapa-De Exercise Room

You are welcome to use the exercise room at our Auburn Health Center. The room has a treadmill, elliptical, stationary bikes, strength training equipment and more. The best part is that it is free to use! There are no expensive gym memberships here.

Before you start, your Chapa-De medical provider needs to confirm that it is safe for you to exercise. Then, our personal trainer will create a custom workout plan tailored to your abilities and goals.

The exercise room is available Monday through Friday by appointment.

Diabetes Prevention Program

The Diabetes Prevention Program (DPP) is a one-year program that helps you feel healthier while lowering your risk of type 2 diabetes. Participants learn about healthy eating, staying active, and managing stress. The program starts with weekly classes for 16 weeks. Followed by monthly sessions to help keep up healthy habits and maintain weight loss. Participants also get free access to exercise equipment and other classes offered within the department.

All Chapa-De patients aged 18 and over are welcome.

Diabetes Management

We are here to help manage type 2 diabetes in partnership with you and your medical provider. Our team can support your health through education, support, and diabetes medication management. Our team of specially trained Diabetes Nurse Case Managers, Dietitians, and Lifestyle Coaches will encourage you to learn about your diabetes and take steps to promote good health.

Through this program, you can expect:

  • One-on-one personalized appointments with a Diabetes Nurse and/or Dietitian.
  • To better understand diabetes and its risks.
  • To feel more confident that you can care for yourself through monitoring, medication, nutrition, and physical activity.
  • To receive support to manage depression, Diabetes “burnout,” social pressures and stress.
  • Regular A1c checks to understand your average blood sugar (glucose) levels.
  • Retinal eye screenings as needed.
