- Treatment of Sprains or Simple Fractures
- COVID-19 Testing & Treatment Prescriptions
In the event that you require services not available at our healthcare centers, we will help coordinate care with a specialist. We can refer you to any specialist in the region if they accept your health insurance plan and if they are accepting new patients.
How To Schedule An Appointment
We want to make your experience in our health centers as comfortable and convenient as possible. Therefore, we want you to have an appointment to be seen. We cannot guarantee that you will be seen if you walk-in without an appointment.
To schedule an appointment quickly and easily, use the Healow app or our online Patient Portal. The Healow app is available on the Apple App Store and the Google Play Store. Use code AHHIAD to get started.

We have same-day appointments that are available on a first-come, first-serve basis. If you would like to schedule a same-day appointment we recommend you call us as soon as we open at 8 a.m. We will see you for whatever you need, whether it is an urgent illness or a regular check-up, during the same-day appointment.
In some instances, we will re-direct patients with urgent problems to the local emergency room or urgent care center. It is best to call us and see if we can accommodate your urgent need prior to coming into our community health centers. However, if you think you are experiencing an emergency, call 911 immediately.
Preparing For Your Visit
All new patients must register before their first appointment, please see How to Register. New patients should arrive at the reception desk 30 minutes before your first appointment to complete the new patient registration process. Established patients should arrive 15 minutes before your appointment time.
You will need to bring the following items to all of your appointments:
- Photo ID
- All insurance cards (Medi-Cal, Medicare or other medical, prescription or dental insurance).
- A list of medication you are taking or bring the medications with you.
- The patient services representative making your appointment will also inform you of any records or information you need to bring with you.
- When bringing children in for immunization, always bring their immunization record with you.
Cancellation, Late & No-Show Information
If you need to cancel or reschedule an appointment please call us. When patients do not cancel their appointment, they deny other patients the opportunity to book that visit. We keep track of the number of “no-show” appointments a patient accumulates and patients who repeatedly miss their appointments may lose the privilege of making appointments at our community health centers.
After Hours and Holidays
Important: If you think you are experiencing an emergency, call 911 or go to the nearest hospital immediately.
Chapa-De patients who are injured, become ill, or have health concerns or questions when our health centers are closed, can still contact us for help!
If our clinic is not open, our answering service will have a highly experienced and caring Registered Nurse return your call. Most calls are returned within 10 minutes. Our nurse will talk you through your symptoms and concerns to help you determine if you can manage your care at home or if a visit to a healthcare provider is needed.
You can also be connected with an After Hours Advice Dentist for dental emergencies or one of our Prenatal Care Providers for pregnancy concerns.
