Licensing & Accreditations
Chapa-De’s health centers are licensed by the California State Department of Health Care Services (DHCS) as community clinics and are certified as Medicare and Medi-Cal providers. Both of our health centers are accredited by the Accreditation Association for Ambulatory Health Care Inc. (AAAHC). Our Auburn health center is recognized as a Level 3 Patient Centered Medical Home by the National Committee for Quality Assurance (NCQA).
