We are dedicated to providing high-quality care regardless of your financial situation or ethnicity. Our passion is to care for American Indian and Alaska Native people as well as low-income individuals and families.

If you are an American Indian/Alaska Native you may be eligible to receive our services at no charge. Chapa-De also accepts Medi-Cal Managed Care Plans, straight Medi-Cal, Medicare and a small number of private health insurance plans. Uninsured patients can also pay directly for the services they receive at our clinics. We offer a sliding scale fee structure and you may be eligible for a discount based on your annual income and family size.

Indian Health Service & Purchased/Referred Care

If you are an American Indian/Alaska Native you may be eligible to receive our services and some medications for little or no cost. To qualify for this benefit, you must be able to provide us with documents verifying that you are American Indian.

We do encourage our American Indian patients to apply for and use other health insurance plans, such as Medi-Cal or private insurance. This allows us to provide more care and services than our limited Indian Health Service monies can cover alone. It also provides coverage for services that are not provided in our community health centers, such as x-rays, specialty care and hospitalization. We do not charge American Indian patients their co-pay or deductible for services that we provide in our community health centers. Some American Indian patients with Medi-Cal from other counties can even convert their plans to one that Chapa-De can accept. Purchased/Referred Care

While there is no charge to American Indians for care at Chapa-De, most services outside of our clinic are not offered free of charge. This includes services provided by medical specialists, hospital stays, emergency room visits and medical x-rays.

Some American Indian patients may qualify for financial assistance to pay for healthcare outside of our clinics. If you think you might be one of these patients, you must have your outside care approved in advance by our Purchased/Referred Care Specialist. If you used outside care due to an emergency, then you must contact the Purchased/Referred Care Specialist within 72 hours of receiving the care. You must use all possible resources, such as private health insurance, Medi-Cal or Medicare before Purchased/Referred Care Services will consider paying for your healthcare bill. Medi-Cal/Medicare

Chapa-De accepts Medi-Cal and Medicare insurance plans. We accept both Medi-Cal Managed Care plans that are available to patients in our area, Anthem Blue Cross and California Health and Wellness, as well as straight Medi-Cal.

Cash/Sliding Scale

Chapa-De offers a sliding fee scale for uninsured and low income patients depending annual income and family size. This discount fee scale is for Non-Indian patients, and is based on Federal Poverty Guidelines. We have a 50% or 75% sliding scale, which is the portion of the charges that the patient must pay.

If you are approved, the sliding scale can be used for medical or dental services. Our sliding scale may not be used for certain items including special supplies, outside lab fees, medical supplies/devices, injections/vaccines, glasses and pharmacy.

Once approved, sliding scale is good from the date you are approved to April 30th of the following year. Sliding scale must be re-applied for every year with current IRS tax information.

Local Programs

Family PACT: Family PACT provides comprehensive family planning services to eligible low income men and women. Family PACT provides family planning services, such as birth control pills, IUDs, vasectomies, and education to low income men and women of child bearing age. Every Woman Counts (EWC): Every Woman Counts provides free clinical breast exams, mammograms, pelvic exams, and Pap tests to California’s underserved women.
