Overview
We invite Chapa-De patients to participate in our group classes and education programs that focus on the prevention and management of diabetes. As a department, it is our mission to help patients establish health goals and provide you with tools and support you need to accomplish those goals. You can take charge of your health and make improvements for you and your family. It is important to remember that making just a few changes can lead to big results.

Diabetes Prevention Program
The Diabetes Prevention Program (DPP) is a year-long program that helps Chapa-De patients to avoid the onset of diabetes and to live healthier. Participants learn about eating healthy, being physically active and managing stress while attending weekly classes for 10 weeks, then 6 bi-weekly classes, followed by monthly sessions to help maintain weight loss and healthy lifestyles.
Participants in the program also get free access to exercise equipment and our lending library with DVDs and books about nutrition and fitness.
The program is open to all Chapa-De patients over the age of 18, who do not have a diagnosis of Diabetes.

Diabetes Wellness Program
If you are living with Diabetes and would like support, Chapa-De’s Diabetes Department is the place for you. Our Diabetes Program staff will partner with you and your medical care team to achieve your goals for health and wellness through self-management education and support with one-on-one sessions with a Diabetes Case Manager, Dietitian, and/or Lifestyle Coach.
Classes are also held periodically throughout the year. Below are some of the topics we cover:
- What is diabetes and how does it affect our bodies
- Checking and understanding blood sugar levels
- Managing high and low blood sugar levels
- How to manage days when you are sick
- Diabetes medications and how they work in our body
- Relationship between blood sugar, insulin and food
- Enjoying healthy eating
- Finding physical activity that will work for you
- Effectively managing diabetes distress, diabetes burnout and/or depression

Diabetes Support Groups
Our diabetes support groups welcome patients living with diabetes and the people who support them. We focus on diabetes self-management in a fun, interactive, supportive atmosphere.
We have a dedicated support group for American Indian and Alaskan Native patients that meets monthly where we provide care in a culturally sensitive manner. We also offer a Type 1 group and a Type 2 group that meet monthly and are open to all Chapa-De patients.

Exercise Groups & Classes
Our Exercise Classes and Certified Personal Trainer are available to Chapa-De patients who are enrolled in our Diabetes Prevention or Diabetes Wellness Programs and/or who are currently seeing one of our Registered Dietitians or Diabetes Nurse Case Managers.

Our exercise classes allow each participant to workout at their own level under the professional guidance of our Certified Personal Trainer. Please make an individual appointment with our Personal Trainer before participating in a group exercise class.
