COVID-19 Vaccine Schedule

Chapa-De will email patients directly when it is their turn for a vaccine. Make sure we have your email address by signing-up for or updating your patient portal account.

When Will the Vaccine Be Available to You?

Below is the Chapa-De Vaccination schedule. This page will be updated on a daily basis with the most current information about which groups are able to receive their vaccinations. Please bookmark this page so that you can easily refer to it.

Vaccinating NOW:

Chapa-De Staff and American Indian Elders

Chapa-De Employees

American Indians 65 Years of Age or Older

Vaccinating NOW:

Group 1

Chapa-De Patients Above the Age of 75 (Including Non-native)

American Indian Patients Who are Healthcare Workers:

  • Paramedics, EMTs, and others providing emergency medical service

  • Home health care and in-home supportive services

  • Community health workers, including promotoras

  • Primary care clinics, including federally qualified health centers, rural health centers, correctional facility clinics, and urgent care clinics

  • Laboratory Workers

  • Dental and Oral Health Clinics

  • Pharmacy Staff

  • Janitorial Staff of Healthcare Workplaces

Vaccinating NOW:

Group 2

American Indian Patients Aged 18-64 with Following Medical Conditions:

  • Cancer

  • Chronic kidney disease

  • COPD (chronic obstructive pulmonary disease)

  • Down Syndrome

  • Heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies

  • Immunocompromised state (weakened immune system) from solid organ transplant

  • Severe Obesity (BMI ≥ 30 kg/m2)

  • Sickle cell disease

  • Type 2 diabetes mellitus

  • Asthma (moderate-to-severe)

  • Cerebrovascular disease (affects blood vessels and blood supply to the brain)

  • Cystic fibrosis

  • Hypertension or high blood pressure

  • Immunocompromised state (weakened immune system) from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, or use of other immune weakening medicines

  • Neurologic conditions, such as dementia

  • Liver disease

  • Overweight (BMI > 25 kg/m2, but < 30 kg/m2)

  • Pulmonary fibrosis (having damaged or scarred lung tissues)

  • Thalassemia (a type of blood disorder)

  • Type 1 diabetes mellitus

American Indian Patients Who are at Increased Risk of Exposure at Work/Living:

  • Education

  • Childcare

  • Food and Agriculture

  • Homeless

  • Incarcerated

  • Transportation and logistics

  • Industrial, commercial, residential, and sheltering facilities and services

Group 3

Chapa-De Patients 65-74 (Including Non-Native)

Vaccinating NOW:

Group 4

Chapa-De Patients 18-64 (Including Non-Native) Who Have One of the Following Medical Conditions:

  • Cancer

  • Chronic kidney disease

All Chapa-De Patients Age 18 and Over (Including Non-Native)

Are You Eligible Based on the "Vaccinating NOW" Criteria Above?

If you are within an eligible group and you can answer "NO" to all the questions below, please call us at (530) 887-2810 for Auburn or (530) 477-1727 for Grass Valley to schedule an appointment.

In the last two weeks have you:

1. Had a fever, cough, shortness of breath, difficulty breathing, sore throat, nasal congestion, runny nose, muscle or body aches, headache, fatigue, nausea, vomiting, diarrhea, chills, or new loss of taste/smell?

2. Tested positive or been in contact with someone testing positive for COVID-19 in last 14 days.

3. Have you received another vaccine?

4. In the last 90 days, have you received monoclonal antibody treatment for COVID-19?

We ask all others to check back frequently and to contact us once you are eligible.
