Chapa-De is here to help you achieve your wellness goals in 2023! New classes for our Diabetes Prevention Program start in February. The free weekly classes help participants to eat healthy, be physically active and manage stress. A few small changes can lead to big results!
With the support of her Chapa-De Primary Care Provider and the Diabetes Program, Julie has lost over 30 pounds and is feeling more comfortable with herself. Our Diabetes Program can help you too!
Diabetes Prevention Program
The CDC has recognized Chapa-De’s Diabetes Prevention Program for outstanding quality and outcomes. This free program is proven to help patients avoid the onset of diabetes, to live healthier, and to feel better while doing the things they love.
Our program offers:
- Nutrition counseling to lose weight in a healthy way
- Exercise counseling modified to your needs and levels
- Help managing stress
- Group discussions to learn from others
- Lifestyle coaches to support you through the program
There are both daytime and evening classes to fit your needs. Classes start:
Wednesday, February 1
Thursday, February 2
5:30pm – 6:30pm
Our program will help you maintain weight loss and a healthy lifestyle all year long. We focus on building healthy habits rather than quick temporary changes. Our program starts with a lot of support and check-ins. You then gradually reduce how often you attend classes as you gain more confidence. You will attend 10 weeks of weekly classes to start. Then you will attend class every other week for 6 classes, before moving to monthly sessions for the remainder of the year.
Participants also gain free access to our exercise room and a lending library with DVDs and books about nutrition and fitness. The program is open to all Chapa-De patients over the age of 18 who do not have a diagnosis of Diabetes and who are not pregnant.
