Introducing our new Electronic Health Record System!
In order to better serve our patients, Chapa-De has upgraded to a new Electronic Health Record (EHR) system called eClinicalWorks. This new system will help us to improve communications with you and offers advanced tools for your Chapa-De care teams.
What is an Electronic Health Record (EHR)?
EHRs are important tools for healthcare providers to keep track of the information we need to properly care for our patients. This is where we take notes about your visits at our Health Centers and where we store important information such as:
- Your vitals information like height, weight, blood pressure, etc.
- Medications that you take and your allergies
- Past immunizations and screening tests
- Current and past health conditions and so much more!
What’s New?
Affordable health care does not mean you have to sacrifice quality. Chapa-De is committed to providing our patients with the best available care, which is why we are upgrading our records system. The new Electronic Health Records system brings exciting changes that improve our ability to serve you. They include:
- Our new system now makes it possible for all of your care teams to see and add the information they need in real-time. This is especially important in Health Centers like ours where you may have multiple providers such as doctors, dentists and psychiatrists who all need to stay up to date on your health.
- The new system communicates electronically with outside entities such as pharmacies and labs, giving you faster results. This means your prescriptions are sent instantly to your pharmacy and that your lab/test results download directly into your health record.
- By having one shared system we can do a better job of coordinating your visits among all Chapa-De departments. If it is helpful, we can try to schedule your next medical appointment the same day as your upcoming dental visit to save you a trip.
- We now offer appointment reminders by text message, automated phone call or email. You can tell us how what is most convenient for you.
- Coming soon, you will also be able to sign-up for the online patient portal where you can view upcoming appointments, request new appointments, view lab and test results, and send secure email messages to your Chapa-De care teams.
