Nutrition, Exercise & Wellness Information/Tips


We want to help you set your own health goals, and then help you work to accomplish them. We would much rather keep you well than treat preventable diseases. We believe that your mind affects your body. We believe that what you eat matters.

We offer classes and groups that can help you achieve your health goals.  Classes are listed and described to the right.

Classes & Support Groups


Diabetes Prevention Program

The Diabetes Prevention Program (DPP) offers services for those American Indian and Alaska Natives patients who are at an increased risk of developing diabetes. This 16 week program provides group sessions and individual support with a Lifestyle Coach, Certified Personal Trainer, Registered Dietician and Licensed Therapist.

During the Diabetes Prevention Program, participants will learn about eating healthy, being physically active and weight management. We also provide a healthy meal at each class along with great teaching tools that you can take home to help assist with your lifestyle changes. You will also have access to our exercise equipment and our lending library full of DVD’s and books on nutrition and fitness. During the course of the program, you will meet other people who are also determined to prevent the onset of diabetes.

The program is currently open to American Indians/Alaska Natives, who are over 18, do not have Diabetes, and have had an A1C of 5.7-6.4

Class Times

January 6 through April 21 | 12 p.m. – 2 p.m. weekly (16 weeks)

January 7 through April 22 | 5:30 p.m. – 7:30 p.m. weekly (16 weeks)

June 1 through September 14 |  12 p.m. – 2:00 pm weekly (16 weeks)

June 6 through September 15 |  5:30 p.m. – 7:30 pm weekly (16 weeks)

DPP After Core Meetings

The Diabetes Prevention Program (DPP) offers services for those American Indian and Alaska Natives patients who are at an increased risk of developing diabetes. After participants have completed the DPP 16 week course, we continue to provide support through our after core meetings that take place the second Wednesday of every month from 5-7 p.m. It is through this continued support and instruction that positive lifestyle changes are maintained.

Diabetes Wellness Program

If you are an American Indian or Alaskan Native patient who has diabetes and would like support, Chapa-De’s diabetes department is the place for you. Our Diabetes Wellness Program has been established for our AIAN population and has been built to provide comprehensive, culturally sensitive care to people living with diabetes.  Our program staff works together to provide Diabetes Self-Management education and support with one-on-one sessions.

Classes are held periodically throughout the year. Below are some of the topics we cover:

  • What is diabetes and how does it affect our bodies
  • Checking and understanding blood sugar levels
  • Managing high and low blood sugar levels
  • How to manage days when you are sick
  • Diabetes medications and how they work in our body
  • Relationship between blood sugar, insulin and food
  • Enjoying healthy eating
  • Finding physical activity that will work for you
  • Effectively managing diabetes distress, diabetes burnout and/or depression

Diabetes Management Support Group

Our diabetes support group is for American Indian or Alaskan Native patients living with diabetes and the people who support them. We provide care in a culturally sensitive manner and we focus on diabetes self-management in a fun, interactive, supportive atmosphere. Our diabetes support group is held the second Tuesday of each month from 3-5 p.m.

Relaxation Class

Each week our class will teach one of four different ways to achieve a deep level of relaxation.

Effortless Breathing | In this class you will learn Effortless Breathing, developed by Dr. Pepper and associates, which is a unique breathing technique directed towards calming stress reactions while increasing relaxation.

Autogenic Training | In this class you will learn Autogenic Training, developed by Dr. Schultz, which is a technique where you repeat certain phrases (call “formulas”) and imagine them to be true while you are in a relaxed position.

Progressive Muscular Relaxation |  In this class you will learn Progressive Muscular Relaxation, developed by Dr. Jacobson, which is a technique where you flex and relax specific muscle groups while you are in a relaxed position.

Self-Hypnosis |  In this class you will learn Self-Hypnosis which is a technique directed towards relaxing the active mind (“conscious mind”) and allow the passive mind (“subconscious mind”) to self-suggest relaxation while you are in a relaxed position.

Classes are held on Mondays from 3:45 to 4:45 p.m. at our Auburn health center. The program is open to all Chapa-De patients but is limited to 10 participants per class, so contact us today to reserve your space!

PACE

PACE is a chronic pain management program that will give you tools to cope with and effectively manage your pain. PACE is a four week evidence-based program aimed at increasing quality of life and improving the overall function of chronic pain sufferers. PACE classes are hosted on-site at our Auburn and Grass Valley health centers. Digital Storytelling Workshops

Coming soon to Chapa-De! Digital storytelling is the practice of “everyday” people telling their story in a compelling, engaging way, using their own words, intermixed with music and pictures for emotional impact.

Stories connect people, have the power to make a difference, can assist with healing, and can lead to positive change. What’s YOUR story?
